Actiq

What is it?

An opiate preparation, Fentanyl citrate, available as white lozenges in 200, 400, 600, 800, 1200 and 1600µg strength. Each marked with strength and name, administered with special applicator for use in the mouth. Tablets are not swallowed.

Medical Uses

Hospital treatment for breakthrough pain in people receiving opiate treatment for severe and chronic pain.

Dosage

Adults: start with lowest, 200µg dose; move applicator around mouth for 15 minutes, if pain is not relieved, add a further 200µg dose. Proceed to higher dose until pain is controlled by single unit dose. The maximum is 4 units daily.

Special Care

Pregnancy, elderly people, patients with head injury, impaired levels of consciousness, elevated intracranial pressure, liver, kidney or lung disease, heart arrhythmia, depression of respiration. Patients must be monitored for depressed respiration. Special disposal of used units required.

Avoid Use

Children, breastfeeding.

Possible Drug and Food Interactions

Cyp3a4 inhibitors, MAOI's, central nervous system depressants, narcotic antagonists, grapefruit juice.

Known Side Effects

Circulatory and respiratory depression, low blood pressure, head ache, shock, dizziness and confusion, vertigo, sleepiness, gastrointestinal upset, itching, dry and ulcerated mouth, hallucinations, flushing and sweating, weakness, breathlessness. Dependence and tolerance may develop. Adverse symptoms must be reported to CSM.
