Clivarine
What is it?
An anticoagulant, low-molecular-weight heparin (LMWH) comprising 1432iu reviparin sodium (anti-Factor Xa activity) per 0.25ml, available as solution in pre-filled vials for injection.
Medical Uses
Prevention of thromboembolism and thrombosis prior to surgery.
Dosage
Adults, 0.25ml by subcutaneous injection given 2 hours before surgery and then repeated once daily for one week, or until patient is able to walk.
Special Care
In diabetic patients, those with chronic kidney failure or metabolic acidosis, there is a risk of adrenal gland suppression which can cause raised levels of blood potassium. Blood potassium levels should be monitored. Blood platelets should be monitored in all patients.
Avoid Use
Children, pregnancy, allergy to heparin, those at risk of bleeding or haemorrhage, those undergoing surgery to CNS or lumbar puncture, patients having spinal or epidural anaesthesia, pregnant women with threatened, imminent abortion, patients with thrombocytopenia (a bleeding disorder in which the number of platelets is reduced), vascular lesions.
Possible Drug and Food Interactions
Dipyridamole, penicillins, tetracyclines, cephalosporins, NSAIDs, sulphinpyrazone, other anticoagulants, cardiac glycosides, aspirin, dextrans, coumarin derivatives, propanol, antihistamines, nitroglycerin.
Known Side Effects
Allergic reactions, thrombocytopenia (bleeding disorder), bleeding (risk with higher doses), short- lived, raised liver enzymes, osteoporosis, if use prolonged, any adverse side effects must be reported to the Committee on the Safety of Medicines (CSM).
