Miacalcic
What is it?
A hormonal preparation affecting bone metabolism available as a solution in vials in three strengths containing 50iu, 100iu, or 200iu of salcatonin per ml. Also, MIACALCIC NASAL SPRAY, delivering 200iu of salcatonin per metered dose.
Medical Uses
Injection, Paget's disease of bone, chronic elevated levels of blood calcium or crisis caused by high blood levels of calcium, pain from metastatic bone cancer, short-term treatment of osteoporosis after the menopause. Nasal Spray, post-menopausal osteoporosis - for long-term treatment.
Dosage
Adults, injection, hypercalcaemic crisis, 5 to 10iu per kg of body weight by slow intravenous infusion with 500ml of saline, given over a minimum period of 6 hours. Chronic high blood levels of calcium, 5 to 10iu per kg of body weight given by intramuscular or intravenous injection either as one dose or in 2 divided doses; continue treatment for no more than 3 months. Paget's disease of bone, 100iu by intramuscular or subcutaneous injection each day at first, reducing to a maintenance dose of 50iu daily; continue for no more than 6 months and Also, consult manufacturer's literature. Pain from metastatic bone cancer, 200iu up to 4 times each day by intramuscular or subcutaneous injection; Also, consult manufacturer's literature. Short-term treatment of osteoporosis, 100iu each day by intramuscular or subcutaneous injection with additional requirement for a minimum of 600mg of elemental calcium and possibly, 400units of vitamin D every day. Children, injection, according to manufacturer's instructions. Adults, nasal Spray, 1 spray each day, ensuring good intake of calcium and vitamin D in diet or as supplements, as advised by doctor.
Special Care
Perform scratch test to exclude allergy if this is suspected, before beginning treatment. Injection, impaired kidney function - blood levels of alkaline phosphatase and urinary excretion levels of hydroxyproline should be monitored and treatment may need to be modified. Spray, inside of nose should be monitored for signs of ulceration and treatment stopped if this arises.
Avoid Use
Injection, allergy, breast- feeding.
Possible Drug and Food Interactions
Injection, cardiac glycosides.
Known Side Effects
Injection, pain at injection site, flushing of face, gastrointestinal upset, rashes, dizziness, anaphylaxis. Nasal spray, local irritation, rhinitis (inflammation of the lining of the nose, with sneezing, mucous production etc), dizziness, gastrointestinal upset, flushing. All adverse side effects should be reported to the Committee on the Safety of Medicines (CSM).
