Mirapexin
What is it?
A non-ergot, dopamine agonist acting upon dopamine receptors available as white tablets in 3 different strengths containing 0.125mg, 0.25mg and 1mg of pramipexole dihydrochloride monohydrate, marked 2, 4 or 6, respectively.
Medical Uses
Parkinson's disease, either as sole therapy or combined with levodopa.
Dosage
Adults, 0.125mg 3 times each day at first, then after every 5 to 7 days, the dose is doubled until 0.5mg is being taken 3 times daily. If needed, a further increase can be made at weekly intervals by increments of 0.75mg each day until optimal response is effected. The maximum daily dose is 4.5mg. If being used with levodopa, the dose of this should be reduced while Mirapexin is being increased.
Special Care
Pregnancy, serious heart or circulatory disease, kidney disorders, psychotic states. Eye tests should be performed regularly, especially in event of changes in vision; Also, blood pressure must be monitored while dosages are being increased. Drug must be gradually stopped - risk of neurolegtic malignant syndrome if withdrawn suddenly.
Avoid Use
Children, breastfeeding.
Possible Drug and Food Interactions
Sedatives, amantadine, alcohol, cimetidine, drugs used to treat psychosis.
Known Side Effects
Dizziness, low blood pressure, sleepiness, sudden episodes of falling asleep (patients should not drive or operate machines), fluid retention affecting extremities, hallucinations, dyskinesia (impaired ability to perform voluntary movements). Any adverse side effects should be reported to the Committee on the Safety of Medicines (CSM).
