Nexium
What is it?
A proton pump inhibitor available as film-coated, oblong, pink tablets in two strengths containing 20mg and 40mg of esomeprazole as magnesium trihydrate.
Medical Uses
Reflux oesophagitis which is causing erosion of the lining of the oesophagus, prevention of recurrence of reflux oesophagitis, treatment of symptoms of GORD. Also, with antibiotics, to eliminate H. pylori and to heal duodenal ulcers caused by H. pylori, prevention of recurrence of stomach ulcers associated with H. pylori.
Dosage
Adults, reflux oesophagitis and erosion, 40mg once each day for 1 to 2 months; prevention of recurrence, 20mg once each day. GORD with no oesophagitis, 20mg once each day for 1 month then 20mg once each day, as needed, for maintenance. H. pylori, 20mg along with lg of amoxycillin and 500mg of clarithromycin, all twice daily for 1 week. Tablets should be swallowed whole or mixed with plain water and drunk, but should not be chewed.
Special Care
Pregnancy, breastfeeding, severe kidney or liver disorders, longterm use. Possibility of malignancy should be ruled out before commencing treatment.
Avoid Use
Children.
Possible Drug and Food Interactions
Other drugs metabolised by CYP2C19, diazepam, phenytoin, warfarin, imipramine, ketoconazole, citalopram, itraconazole, clomipramine.
Known Side Effects
Dry mouth, gastrointestinal upset, skin effects, headache. In rare cases, anaphylaxis, angioedema (widespread swelling due to fluid retention).
