What is it?

An ACE inhibitor available as capsules in 3 strengths all containing trandolaprii. Yellow/red capsules contain 0.5mg; orange/red capsules contain 1mg; red/red contain 2mg.

Medical Uses

Dysfunction of the left ventricle following heart attack, mild to moderate high blood pressure.

Dosage

Adults, 0.5mg once each day at first (starting 3 days after heart attack, if being used for ventricular dysfunction), increasing every 2 to 4 weeks to a maximum of 4mg as a single daily dose. Maintenance dose is in the order of 1 to 2mg once each day. Any diuretics being taken should be discontinued 2 or 3 days before treatment starts, but can be resumed later, if needed.

Special Care

Liver or kidney disease, kidney dialysis, bilateral renal artery stenosis (narrowing of both renal arteries), undergoing anaesthesia or surgery, congestive heart failure, patients with low fluid and salt levels. Kidney function should be monitored before and during treatment.

Avoid Use

Pregnancy, breastfeeding, obstruction of blood outflow from heart or aortic stenosis (narrowing of aorta), angioneurotic oedema (widespread swelling due to fluid retention) caused by previous ACE inhibitor treatment, hereditary or of other cause.

Possible Drug and Food Interactions

NSAIDs, potassium- sparing diuretics or supplements, TCADs, lithium, neuroleptics, antihypertensive drugs, antidiabetic agents.

Known Side Effects

Rash, cough, weakness, headache, dizziness, palpitations, hypotension. In rare cases, agranulocytosis (a blood disorder characterized by abnormal reduction in number of white blood cells (granulocytes), angioneurotic oedema, depression of bone marrow.
