Pegasys
What is it?
A preparation of pegylated interferon (rbe) available as a solution in pre-prepared syringes in 2 strengths containing 135µg and 180µg of peginterferon alfa-2a(rbe) per 0.5ml.
Medical Uses
Combination treatment with tribarvirin (unless contraindicated or intolerant), for hepatitis C in patients positive for HCV-RNA and who have high transaminase levels.
Dosage
Adults, 180µg by subcutaneous injection into thigh or abdomen once each week combined with tribarvirin taken by mouth. Treatment to be continued for 24 or 48 weeks, depending upon patient status. Doses to be reduced if symptoms of intolerance occur or in event of neutropenia or thrombocytopenia. Also, manufacturer's literature should be consulted and drug administered under specialist supervision.
Special Care
Psoriasis, autoimmune disorders. Thyroid, blood and biochemical tests required before and during course of treatment. Moderate kidney or liver disorders - monitoring required and dose reduced or withdrawn if condition worsens. ECG to be performed if existing heart disease present. In event of loss or deterioration of vision, eyes must be examined. Patients should be asked to report symptoms of depression, especially those at possible risk of suicide. Drug must only be administered by specialist in the treatment of hepatitis C.
Avoid Use
Pregnancy, breastfeeding, previous psychiatric disorder, history of severe heart disease, decompensated cirrhosis, serious liver disease, autoimmune hepatitis.
Possible Drug and Food Interactions
Muscle suppressants, immunosuppressants, theophylline.
Known Side Effects
Fever, gastrointestinal upset, muscle and joint pain, rigors, fatigue, anaemia, 'flu-type symptoms, headache, depression, skin effects, insomnia. Any adverse side effects should be reported to the committee on the safety of medicines.
