Tildiem LA
What is it?
An antianginal and antihypertensive preparation and class III calcium antagonist, available as sustained-release capsules in 2 strengths containing diltiazem hydrochloride. Grey/pink contain 200mg and yellow/ white contain 300mg. Also, TILDIEM RETARD, sustained-release, white tablets in 2 strengths containing 90mg and 120mg of diltiazem hydrochloride. Also, TILDIEM TABLETS, off-white tablets containing 60mg of diltiazem hydrochloride coded TILDIEM 60.Usedfor: angina, mild to moderate high blood pressure. Tildiem tablets are for angina only.
Dosage
Adults, Tildiem L A, 1 x 200mg to start taken before or with meal once each day. Then increase if required to 2 x 200mg tak en in same way once each day or to 1 x 300mg. The maximum daily dose is 1 x 300mg plus 1 x 200mg once each day . Elderly patients, start with same dose taken in same way then increase if required to1 x 300mg once each day. Tildiem Retard, angina, 1 x 90mg or 1 x 120mg twice each day to start, increasing if required to a maximum dose of2 x 120mg twice each day . Elderly patients, 1 x 60mg tablet twice each day to start, then increasing to 1 x 90mg or 1 x 120mg twice each day. For high blood pressure, 12Omg twice each day to start then 2 x 90mg twice each day, if required. Elderly, 120mg each day to begin with then 120mg twice daily, if required. Tildiem tablets, 60mg 3 times each day to begin with, increasing if required to a maximum dose of 480mg each day as divided doses. Elderly, 60mg twice each day to begin with then increasing if required, as directed by physician. Special cure: check heart rate in elderly or cases of liver and kidney disorder. Monitor patients with mild bradycardia (slow heartbeat) or prolonged PR interval (part of ECG).
Avoid Use
Children, pregnancy, breastfeeding, serious bradycardia, heart block, sick sinus syndrome, failure of left ventricle.
Possible Drug and Food Interactions
Other antihypertensives, β-blockers, heart depressants, digoxin, dantrolene infusion, cyclosporin, lithium cimetidine, theophylline, carbamazepine.
Known Side Effects
Swollen ankles due to fluid retention, 1st degree heart block, rashes, slow heartbeat, headache, nausea.
