Yutopar
What is it?
A B-agonist for relaxation of uterine smooth muscle available as scored, yellow tablets containing 10mg of ritodrine hydrochloride marked YUTOPAR. Also, YUTOPAR INJECTION a solution in ampoules containing 10mg of ritodrine hydrochloride per ml.
Medical Uses
Premature labour (with no complications) between 24 to 33 weeks; to delay delivery in order to administer drugs or other measures.
Dosage
Women injection, 50µg per minute given intravenously with 5% dextrose solution to start then gradually increase dose to control. The usual range is between 150 to 350µg per minute. Also, consult manufacturer's literature. Tablets, to be given after injection to maintain control; 1 tablet 30 minutes before end of intravenous treatment then 1 every 2 hours for 24 hours. Then, 1 or 2 tablets every 4 to 6 hours as directed by specialist.
Special Care
Diabetes, suspected cardiovascular disease of the mother, multiple pregnancy, high blood pressure, overactive thyroid gland, infection in mother, diabetes. Monitor heart rate of mother and foetus. Fluid overload - hydration of mother should be monitored and fluids given intravenously kept to a minimum level.
Avoid Use
If a prolonged pregnancy would be hazardous, eclampsia and pre-eclampsia, heart disease in mother, cord compression, antepartum haemorrage (bleeding before the birth and after 28th week), death of foetus, chorioamnionitis (inflammation of placental membranes). Possible interactions: β-blockers, MAOIs, sympathomimetics, potassium-depleting diuretics, anaesthetics, corticosteroids.
Known Side Effects
Anxiety, rapid heartbeat, tremor, raised blood-sugar levels, impaired liver function, rise in liver transaminases (enzymes), nausea, heart arrhythmia, chest pain, flushing, sweating, blood changes - leucopenia and/or agranulocytosis, increased secretion of amylase (enzyme), enlargement of salivary glands. In the event of pulmonary oedema in the mother, stop drug immediately and administer diuretic.
